Your First Visit

Your first visit to Sam’s Physical Therapy, P.C. in Mason, Leslie or Charlotte, Michigan involves a comprehensive evaluation and diagnosis followed by your first treatment in normal situations.

Please arrive at least 15 minutes before your scheduled appointment to allow for your medical history to be taken and your file opened. Bring any notes or images from your doctor as well as a list of prescribed and over-the-counter medications you take regularly.

Please wear comfortable clothing so you will be at ease during your therapy session.

Be sure to bring your insurance card as well as a photo ID.

Monday to Friday 8 a.m. to 5 p.m.
We know our patients lead busy lives and we do our best to be flexible in accommodating all schedules if possible.
